We are looking forward to supporting your student's learning and development.

The purpose of a speech pathology screener to screen for the presence of a delay/disorder of speech, language, literacy, voice, fluency, social communication within 30 minutes. Each student's skills within these areas will be reported on as 'within typical range', 'requires monitoring' or 'needs formal assessment' (and intervention).

Please highlight all statements in which you have some concerns for your student's development There is a section at the end to further detail relevant information. We rely on your information to provide context to your concerns from a classroom perspective. It assists our assessment process and quality of the outcome report.
